  2. List of equipment
  1. Copy of your valid certificate of General Liability Insurance with a minimum of one million dollars - Full page & Acord Certificate
  2. Copy of your valid driver’s license(s)
  3. Copy of your US certifications – (Ex: WA, NY, NC, VA, etc.)
  4. Pictures of each vehicle
